Pacific Diagnostic Laboratories is contracted with a wide range of insurance companies and also offers discount options for uninsured. Please see the Frequently Asked Question section for details.
When you chose an insurance plan you entered into a contract that required you to pay co-insurance and deductibles at certain rates. We are billing you at those rates based on the EOB (Explanation Of Benefits) we received from your insurance.
Can I order my own lab tests at PDL?
In alignment with the California Business and Professions Code Sec. 1246.5, PDL offers the following as self-directed (self-orderable) tests:
- Lipid Panel (Cholesterol, triglycerides, LDL and HDL)
- Glucose
- Hemoglobin A1c
- Occult Blood
- Pregnancy Test (urine)
- Urine Dipstick
Please note:
*Minors cannot order tests on themselves.
*Patients cannot combine physician/client requisitions with self-directed requisitions.
*Self-directed testing must be paid for at the time of collection.

How can I find out if my insurance company has paid this claim?
Your insurance company will provide an eligibility of benefits (EOB) document with details of what has been covered on your claim. We recommend you contact your insurance company for any questions related to coverage.
I do not have insurance, what options are available?
PDL offers a Time of Service (TOS) program that provides discounted pricing for most laboratory tests. To obtain this discount services must be paid for at time of check-in.
If payment cannot be made at the time of service, PDL offers a Prompt Pay discount which can be obtained by contacting our Billing Department at 866-809-9048 once you receive your bill (Note: Prompt pay discount is only available within 30 days of receiving your bill).

I do not have insurance, do I have to pay upfront or can I be billed?
If you are unable to pay up front, PDL will bill you for the testing at regular rates. A prompt pay discount may be available by calling our Billing Department at 866-809-9048 when you receive the bill.
What insurance does PDL accept?

Why didn't my insurance pay for my claim?
Each insurance company processes claims differently. You will receive an Explanation of Benefits (EOB) from your insurance company that details the services and whether the services were eligible for coverage. If you need additional assistance in determining why your insurance did not pay on your claim, please contact your insurance carrier directly.
Why do I need to provide billing information every visit to PDL?
To ensure accuracy of coverage, PDL requires a verification of billing information on each visit including obtaining a copy of insurance card(s).
Why did I receive a bill from PDL?
You will receive a bill from PDL if your ordering physician did not provide your full insurance or billing information or if the remainder of the balance is a patient responsibility portion for testing services (co-pay, deductible, share of cost).
